* Patients are eligible for Hand Therapy services inclusive of the wrist, elbow, and thoracic outlet problems. You must obtain a referral from your primary doctor.

** Patients are eligible for Hand Therapy services only. You must obtain both a referral and out of capitation authorization letter from your primary doctor.

We will make every effort to verify your insurance benefits for you prior to your first appointment. If we are not a provider for your insurance, we will offer private pay arrangements or help direct you to a participating provider.
